Tests to be conducted - 

1. Mammography
2. PAP Smear
3. Ultra-sonography (Pelvic & Abdomen)

at ALPHA DIAGNOSTICS, Tilak Road, Pune

Project Rules & Guidelines : 

1. Recommended Age : 40 yrs & above
2. Valid Phone Number - Providing a valid Phone Number is mandatory. All the reports will be shared by Alpha Diagnostics centre over provided contact details. (Note : Keeping in mind individual's privacy, reports will not be shared with Umeed Cancer Support Forum) 
3. Hard Copies of the reports can be collected from Alpha Diagnostics , Tilak Road
4. Only in case of unavoidable circumstances, change of timeslot will be allowed & any such change in date and/or timeslot needs to be coordinated with Alpha Diagnostics team.
5. Each participant agrees that Umeed Cancer Support Forum team have the right to all data submitted by the participants and may at their sole discretion share the same to our Alpha Diagnostics for the purpose of managing the tests smoothly.
6. Team Umeed Cancer Support Forum reserve the right to cancel the event or postpone the date owing to conditions beyond the control of the organisers like pandemic, epidemic, civil riots, terrorist strike, floods, cyclone, natural disasters, public unrest, etc and such a situation shall be treated as force majeure conditions.
7. By registering, the participant agrees, accepts and acknowledges the terms & conditions of the event and waive all claims against the Umeed Cancer Support Forum and any party acting on its behalf.
